CONFIRMATIONS
Receipt of each registration will be acknowledged with a mailed confirmation letter. If you do not receive a confirmation letter prior to the event, please call 517-336-7585 to confirm your registration.

LATE REGISTRATIONS
In order to ensure adequate space and materials for participants, advance registration is required. To register less than 10 days in advance, please register by phone.

REFUND POLICY
MSTCVS understands the unpredictable schedules of physicians. However, due to meeting expenses incurred, it is necessary to charge a $75 cancellation fee. Participants must cancel at least five (5) business days prior to the event. Refunds will not be provided after the five (5) day cut-off but replacements are always welcome.

Hotel Information

The historic Park Place Hotel invites you to explore some of the features that make them unique, such as the downtown Traverse City location, history, gracious accommodations, thoughtful amenities and comprehensive services.  The Park Place hopes you’ll see why they are such a special place, ready to be your home away from home when you visit the area.
To receive the special MSTCVS rate of $199.95 (plus fees and taxes), reservations must be made by July 7, 2010.
Reservations can be made by calling the Park Place at 231-946-5000.

Exhibitor Information/Policies

ELIGIBILITY FOR EXHIBITING: Exhibits should be staffed during all exhibit hours by qualified persons who are able to explain or demonstrate the displayed items.

ASSIGNMENT OF SPACE: Exhibit space is allocated by MSTCVS staff.

BOOTH DESCRIPTION: Each exhibit will include one six-foot, skirted table with two chairs, electrical hook-up, and two continental breakfasts and lunches (four for sponsors).

ELECTRICAL NEEDS: Electrical service for one outlet is provided. Additional requests must be made prior to the conference to Angie Kemppainen, Executive Director, at 517-336-7586.

LIABILITY/INSURANCE: Neither MSTCVS nor Park Place Hotel assume responsibility for any loss, injury or damage occurring to an exhibitor or any exhibitor’s property. It is the exhibitor’s responsibility to have their person/materials insured against theft, damage, loss or injury.

EXHIBITOR REGISTRATION: All exhibitors must register at the MSTCVS registration desk before setting up exhibits.

HOTEL RESERVATIONS: Overnight guestroom reservations must be made directly with the hotel of your choice.

CANCELLATIONS & REFUNDS: Cancellations will be accepted with written notice on or before July 2, 2010. After July 2, 2010, a 50% cancellation fee will be assessed.

SET-UP AND TEAR-DOWN: Set-up time will be Thursday, August 5, 2010, from 4:00 – 6:00 pm. While exhibitors are encouraged to mingle with participants during all meals, dedicated session in the exhibit hall will occur at the following times:

Booths may be torn down after 3:00 pm on Saturday, August 7, 2010.
